Creating a new Patient is simple in EZClaim!

To open the patient screen, click on the 'Create Patient' button located on the home screen quick access tree or the 'Patient' button on the Home ribbon bar.

The patient demographic screen will be shown, giving you the chance to enter all the necessary patient information.

Entering Patient Information

Do not use initials or credentials. MR., MS., DR., MD, INC. etc.

Do not use words such as ‘SAME’ or ‘NONE’ or ‘N/A’.

    Tip: When you're entering the patient's address, simply input the first letter or two in the City ST Zip field.

    This will bring up the City/State/Zip Library, which automatically stores all previous entries (unless you've turned off this feature).

    You'll then see a drop-down list based on the characters you entered. Just select the appropriate option from the drop-down, and all three fields will be populated.

    The cursor will still be in the City ST Zip field. If you tab out of that field, the cursor will move down to the next required field, which is DOB.

    • Use the expander buttons to view additional information
    • Optional:  Patient Classification, Claim Template, Copay Amt
    • Enter Diagnosis codes on the Patient Information screen only if codes remain the same for all dates of service
      • For electronic claims, up to eight Diagnosis codes may be entered
      • Claim-specific codes will be entered on the Charges screen
    • Enter Patient Contact Information for your records
      • This information will not be sent with a claim
    • Enter previously set up Physician/Facility information; Billing Provider, Rendering Provider, Facility, and/or Referring Provider as required by your Payer
    • Use the expander button to open Additional Claim Information
    • Use expander buttons to add additional Patient information as required by your Payer
