Claim Templates

EZClaim has a variety of Claim template options, review this article to learn more

Tip: You can create new Claim templates by accessing the Claim Template Library. 

Previous Claim

The Previous Claim template functions as follows when accessing the new claim screen (or switching to the <Previous Claim> template):

  • The previous Diagnosis Codes values will be loaded.
  • The previous Date of Current will be loaded.
  • The previous Physician Library Entries will be loaded if Yes is clicked when prompted.
  • The service line preview row will be disabled

By selecting a Calendar date, EZClaim will automatically populate the service line(s) from the previous claim. It is important to note that the previous claim refers to the most recently entered claim for the patient and may not correspond to the claim with the most recent date of service.

Important: For Institutional claims, such as Type of Bill and Admitting Diagnosis, you will need to populate these categories on a claim-by-claim basis. To make it more convenient, you can populate these categories under Initial Claim Values at the individual patient level.

Alternatively, if these fields are consistent across a wide range of patients, you can populate them at the Patient Template level. Patient Templates can also be applied to existing patients as needed.

Previous Service

The data from the previous service line on the last claim will be automatically transferred to the Charges screen. If necessary, you have the option to modify the information in the Preview line before selecting the calendar dates.

Note: When the Previous Service template is selected, you may see the following pop-up. Click Yes if you would like to replace the physician/facility information with the patient default information

Custom Template

A Custom Template enables users to save specific claim information that is frequently entered. For instance, if the same two or more services are performed during every first visit, users can save those services under the name Initial Visit. When a patient comes in for an initial visit, users can simply access the Initial Visit template, select a date, and the claim will be completed automatically.

  • Open a claim and enter the service line items to save for the Template (Do NOT select a service date from the calendar)
  • Click on Save as a Template from the top ribbon bar
  • When asked to Save changes, click Yes
  • Enter a Name for the Template

The template will now be in the Template Library and available to select on all new claims.

When applying a custom template, EZClaim will perform a procedure code lookup to retrieve the charge and other relevant information.

Note: The procedure code lookup for custom templates operates differently than a manual entry procedure code lookup. Specifically, for claim templates, the procedure code lookup will populate the following fields based on the data found in the Procedure Code Library:

Charge

•Allowed

•Adjustment

•Cost

NDC (Drug) Code, Drug Unit Measurement, and Drug Unit Count – If the claim template does not have a value, then all three of these fields will be populated from the procedure code library.

Revenue Code – If no value is included in the claim template.

Patient Amount Due - If the Set Patient Amount Due is checked in the procedure code library, the program will enter a patient amount due based on their patient record.

The lookup will not set the following fields.  These fields must be set in the claim template:

•Modifiers

•Product Code

Previewing/Editing Claim Template Data

  • Click on to open the Claim Template Library
  • Select Template to edit or preview
  • Make desired changes
  • Click on Save & Close to save changes
